Before assessment, treatment or exercise prescription can begin referral from your animals veterinarian must be obtained, this is a legal requirement for any Veterinary Physiotherapist.

 

Please print off the referral form and provide as much information as possible before asking your vet to sign their consent. This may then be emailed to handhvetphysio@outlook.com before your appointment.

 

Alternatively it may be handed to Harriet at your initial assessment.
